Notice
The Appearance form has been revised for
January 1, 2012.
As a result of changes to court rules (sections 3-3, 3-4 and 3-5 of the
Connecticut Practice Book) that went into effect on January 1, 2012, the Appearance form,
JD-CL-12, has been revised. For appearances filed on or after January 1, 2012, the revised
version of the form must be used. It is available at https://www.jud.ct.gov/webforms/forms/cl012.pdf.
