Foreclosure Matters, Affidavit Federal Loss Mitigation Programs

The revised Affidavit Federal Loss Mitigation Programs form (JD-CL-114, Rev. 3/12) must be used to comply with the Mortgage Foreclosure Standing Order Federal Loss Mitigation Programs, form JD-CV-117, on and after March 1, 2012. 

The revised form is available on the Judicial Branch forms website. The revised form must be used for any such affidavit that is filed with the court on or after March 1, 2012.
