While the Discrimination Complaint / Federal Grants Form (JD-ES-284) and the procedures for responding to discrimination complaints refer to programs receiving U.S. Department of Justice funds, these documents can be used for programs involving both U.S. DOJ and U.S. DOT funds. The Discrimination Complaint / Federal Grants Form (JD-ES-284) may also be used for claims of discrimination based on income-level or Limited English Proficiency.
